Seems simple, but if I had a dollar for every leash that was accidentally dropped by someone who was confident they had hold of their dog - I'd be ever so slightly less broke. For the big dogs, the unpredictable dogs, or the feisty dogs that like to throw their weight around - the way you hold that leash can make all the difference between you letting go or managing to hold on when something takes you by surprise.
